Update the distributed HTML documentation where needed for GS-ST v0.3.1.  In
particular, update the developer guide, which contains some incorrect
information.

The following documentation changes have been made:

- A more detailed discussion of the issuing credential (EEC or proxy) has been
added to the User Guide.

- The phrase "TeraGrid Science Gateway SAML Extension" is now used consistently
throughout the documentation.

- A new section on "Deploying the SAML Tools into Tomcat" has been added to the
TeraGrid wiki doc.

Also documented the new configurable bootstrap properties in the install notes.

Committed the updated HTML docs (plus the CHANGES files) to CVS HEAD.
