Allow for authentication via Myproxy username and password. When combined with
in-browser certificate issuance (Bug 6637) this will allow for MyProxy
certificate issuance directly to the browser.

Pruning list required for 2.0 release based on what is needed for CILogon
service and this is not. Will do for post 2.0 release.

I believe this will take enhancements to the Globus::MyProxyClient module as it
only supports certificate-based authentication now (am 90% sure of that).
